I would always choose the islands over any mainland areas. Penang Island can even be reached by car (though beaches there too aren't that stunning compared to other islands).
If you want real tropical beaches and crystal clear waters, head to islands like Perhentian, Redang, Kapas, Tioman etc. If you want beautiful beaches with interesting inland and beautiful kampungs, go visit Langkawi or Pangkor.
As Borneo is also part of Malaysia, you might want to check out the beaches near Kota Kinabalu (stunning) and the beaches near Kuching (Damai Beach, stunning too!).
